TL;DR: wpa_supplicant gets PREV_AUTH_NOT_VALID message from AP, should
immediately reauthenticate to same AP, instead blacklists AP and
attempts to find different AP with same ESSID. If there isn't any,
association permanently fails.
This is evidently fixed in wpa_supplicant 2.5; all Ubuntu releases up to
and including Zesty are on 2.5.
