Title:
  USB Headset randomly stops working

Status in pulseaudio package in Ubuntu:
  Incomplete

Bug description:
  Sometimes, sound output to my USB headset (MS Lifechat LX3000) stops
  working. When this happens the headset still shows up as an output
  device under sound settings, but selecting it as output makes no
  difference, sound just keeps coming out of my laptop's speakers in
  every application.

  I have to run pulseaudio -k (or login/logout) when this happens to get
  output working again. I haven't been able to reproduce exactly when
  this happens at all, it seems totally random so far.

  I'm running precise 12.04 with all updates, but I also experienced
  this exact issue in 11.10.
