Fresh 18.10 install, everything updated. Happens quite often, but not always (30% ?).
I have installed following extra extensions: Unite, Caffeine. Nvidia Graphics driver is installed. It looks exactly the same as in @korg's photos, including previews. Apps can be started by clicking.
