The topmost icon is always missing (with the exception below) and replaced by a small empty square (which can still be clicked to open the application). This is very reproducible; I have the same issue on two separate machines, and the behaviour is identical * Opening the dash shortly after login prevents the problem from happening - all icons display correctly and remain correct for the duration of the session. * Waiting a while (e.g. one minute) after logging in before opening the dash will always result in the topmost icon being missing, and the only way to get it back is to restart the session. * It doesn't matter what application is placed in the topmost position on the dash - its icon will go missing. This doesn't seem to be related to extensions as I have extensions turned off in Tweaks, and both X11 and Wayland sessions have the same behavior.
