Hi, I have a fresh installation of Ubuntu 20.04 with Gnome Image Viewer version 3.36.2 and I have the same issue.
When I rotate a single image and exit, a confirmation dialog appears and clicking save result in the right behavior. Nevertheless if i rotate multiple images and then exit the program modify the images but finally they are not rotated at all. You need to at least 2 images in a directory 2. Open first image in Image Viewer and rotate the image 3. Move to next image 4. Close Image Viewer. It will ask if you want to save changes to first image and you say YES. 5. Open first image again and you'll see that the rotation change has not been saved. If however you open any single image in Image Viewer and rotate the image and close Image Viewer without moving on to another image, then it will again ask you if you want to save changes to the image. But this time it will actually change the image orientation. This probably applies to other image changes but I haven't tried it.
