(a) The version is correct:

GNOME Shell 3.38.2

nemanja@thinkpad:~$ dpkg -l | grep gnome-shell

desktop

(b) I'm not sure what do you mean?

Could the faulty GPU memory be also responsible for such behavior?
Randomly, I'm experiencing random graphics crashes (also on previous Ubuntu 
19.04) and I plan to do a GPU memory test.

Also, maybe related:
Enabled Gnome extensions:
- Dash to Panel (https://extensions.gnome.org/extension/1160/dash-to-panel/)
- Desktop icons (system extension)
