Like Max Gardner this happens to me sometimes and (so far) only when opening files through nautilus. The affected LO instance doesn't show up on Alt+Tab or the Unity launcher. Sometimes it opens just fine. Maybe it's related to the lo-menubar package? It's very hard to reproduce consistently as it appears to be independent of file type or contents. Also, after opening and closing files a few times the problem seems to go away for the duration of the session.
