Updated patch which doesn't create a new endpoint -- just uses the existing 
{{tika/form}} and {{meta/form}} endpoints.

There are two forms at the top of the welcome page. Both allow you to upload a 
file. One grabs metadata and one grabs content.

I'd close this as a duplicate of TIKA-1269, but I'm not sure how we'll 
integrate form submission with Miredot.
