Unexpected exceptions in async clients or services can get swallowed

If an exception happens while processing a asynchronous client or service
invocation where the code is running on another thread then in some code paths
the exception is caught and ignored so there is no indication anywhere that
anything has gone wrong. This can make debugging the runtime issues can't
difficult.
