In August, Mozilla hosted an event for some people interested in "fixing appcache". The raw notes from that day are here:

The basic take-away message is that appcache is usable as it stands now but has a few issues that need to be improved for it to be widely adopted (and loved, of course). Representatives from Gecko, Chrome, and IE agreed on what needs to be fixed and we're just waiting on a concrete proposal to get started. One of Mozilla's WebAPI team's goals for early in 2013 is to come up with this proposal. We had hoped to get to this before the end of December but B2G v1 takes priority.
