I've updated offliner and now it's possible to configure it in such a
way it will download a zip of the app and use its contents to populate
the cache as Benjamin Kelly suggested.
You have more info at [1]. Notice, it now depends on the zip.js library
but it is still possible to collapse all the sources in only one keeping
the advantages of "include these only files and voilà".
I'm thinking about the update process. Something related with E-tags and
updating channels providing up to date packed apps.
Furthermore, I missed some useful hooks and functionalities in Service
Workers proposal as the ability of force an update or some event that
triggers each time it starts running (not when installed or activated
but when it starts to run).
It could be great if SW could start other workers too. :)

A website-in-a-zip will be a single network request, which might perform far
better under some network conditions than many network requests.
You can implement this in Service Workers. Intercept the fetch events for
individual resources, make a single fetch() for an archive blob, split it up in
the SW, stick the resources in the Cache individually. Then subsequent fetch
events find the individual resources in the Cache. (Similar to the Hacks post
I'm working on.)
