I copied the sample cow.pdf into the local directory as cow_brl.brl, and saved 
the below as test.tex.  The resulting pdf is attached.  With my patch the 
graphic is included.

  \definegraphictypesynonym[brl][pdf]
  \starttext
  \externalfigure[cow_brl]
  \stoptext
