I think I'm doing roughly what you are.  I'm accepting the default
response if there is one, selecting Cancel if there's a choice between
OK and Cancel, or otherwise doing nothing.  But I don't believe my
prompt service is actually being used.

Please can you post (or point to) the code you use for installing
the prompt service.
