Add DAM dialog fields for iimage name and type

Add two fields on the DAM upload dialog:
- one for the file name (property name: name)
- one for the type (property name: type)
In addition add a hidden field that change the DAM node name. Currently, the
DAM node name is equivalent to the original file name (same behavior as for the
previous DMS)
