Description: When you click on the "Select New..." button, the page chooser
is displayed. Then when you click on either "Cancel" or "Choose" The page
chooser closes, but the Teaser dialog is now corrupted. The form is not
visible, and the "Cancel" and "Save Changes" buttons are near the top of the
