- remove FormDialogPresenterFactory, now injecting the presenter
- move DialogDefinitionRegistry into FormDialogPresenter, passing dialogName
- remove reference to form for validation, now injecting the validator
- instead of passing the FormDialogPresenter into the actions to get the FormPresenter to validate the form, we now directly inject a validator interface, which is implemented in FormDialogPresenter
