Description:

After the dialog and form refactoring - some things are named incorrectly, and there are some styles hanging around that are no longer used.
Key.
There is no longer a form-header, only a dialog-header.
There is no longer a form-footer, only a dialog-footer.
So change java classes to no longer reference form-header.
Delete un used form styles.
Move and rename styles in form.scss to dialog.scss
