Microsoft calls them "cut-ins" and they are defined in the
MATH table. They are intended for sub- and superscript (ie,
apparently not for general combinations of characters), and
may be specified for each quadrant (each quadrant may have
multiple cut-ins). Lines are horizontal and vertical.
