There's a bug in the mode 0 support. See bug 1097524. This seems to have gotten sidetracked because of a lack of good mode-0 endpoints to test against. I'll check the patches there for bitrot and then you can see if that fixes it for you.

However, I would suggest moving to mode 1, which is what most equipment prefers. Generally mode 0 is old, old equipment.
