The date you put into the required date field for Action 1a and 1b should represent all of your included root certs. So, if most of your SHA-1 TLS cert issuance stopped in December of 2015, but for one of your included root certs you issued SHA-1 TLS certs on April 13, 2016, you would enter 04/13/2016 for your answer to ACTION 1a. Then you would clarify in the "ACTION 1a TEXT INPUT" section.
