With forum-level subscriptions working now, I'm thinking we should remove this
thread-level subscription UI http://screencast.com/t/5KuVWoc16 It's ugly. If
we want to keep thread-level at all, we could have the standard subscribe icon
in the header of an individual thread, but no mass subscribe UI.
