There is indeed an issue with new threads that are moderated.  After approval 
the thread doesn't show up the forum page.

I'm not able to reproduce what he's seeing, I don't know if it's just a 
difference in the particular steps involved, or if it's a forum specific issue.

In any case though, there's definitely some odd behavior going on here. It 
looks like the post did actually appear, but it's at the bottom of the list 
instead of the top. His reply though, nowhere to be seen, though it does say 
there are two posts on the thread list.
