Create Slider App wizard is to support new features (Scheduler, Labels, Log
aggregation). The wizard needs to have new UI elements to handle these.
Screenshots have been attached for changes in 2 pages. The UI elements need to
be created only - the backend hookup will come later.
Second page has two hovers (shown in blue) which should show up entry into
field.
