Fix For: 2.0.0
* For alert instances, we show text as {{service : host}}. Instead we want to
show {{service / host}}. If either host or service is missing, we dont want the
separator.
* Alerts popup has columns that are not aligned
* Alert instances have last-triggered using the wrong timestamp
* Small style fixes in alert-definitions table and alert-definition page
