Summary: Rolling Upgrade: inconsistent statuses

1. The currently executing upgrade group goes back and forth from IN_PROGRESS
to PENDING state.
2. Completed upgrade groups should have COMPLETED state instead of PENDING.
3. When the upgrade is paused, overall progress shouldn't be at 100%. The
percentage completion should remain as is.
