is this a defect-issue (the Dev's Guide is wrong), or a RFE (the
implementation should be modified to support this feature: using
XDocumentInsertable::insertDocumentFromURL passing an InputStream)?

for me it's a defect that the method doesn't support the MediaDescriptor.
