RecursiveBundleTracker doesn't work across isolated Regions

RecursiveBundleTracker only works on isolation-free frameworks or with the
obsolete and never-official equinox composite bundles. In order for extenders
such as the BlueprintExtender to work with isolation we need a way to track all
bundles, not just ones in the same isolation unit.
One easy way to implement this is to install a bundle event hook and use it to
drive a bundle tracker.
