the failures are not the ones we are interested in :(
what i can tell from the **info** log on failure is that it looks like
`ON_FIRE` is being set *before* the `ServiceReplacer` starts to run. so the
logging you add here won't help; we know why there is only one entity, the
`ServiceReplacer` has not run.
i've created some other logging in #213 to help investigate that.
