New patch containing fixed:
- use guava mapmaker (weakValues) instead of WeakHashMap
- Store default graph name in index (as default + as graph)
- simplified UriRefSet (see previous bullet)
- proper index retrieval (as MGraph instead of Graph)
- use defaultModel (instead of separate named graph) for storing index.
