$scope.fileLoadFailed is set using a Boolean:
    $scope.fileLoadFailed = true;
    $scope.fileLoadFailed = false;

but the conditional treats it as a string and always fails:

     if($scope.fileLoadFailed === 'false'){

Update comparison to compare as a boolean.
