Is it an upgrade, or a fresh install? Which OS? Installed via package?
Generally this points to something during the startup not finishing, so you
may want to scroll up well beyond the actual login attempt to see if
something failed. I believe I saw this once when it couldn't find the cloud
user's home (since it changed to /var/cloudstack/management) or wasn't
owned/writable by cloud to set up ssh keys there, and maybe once during a
botched DB upgrade.
