    CLOUDSTACK-6928: fix issue disk I/O throttling not applied

    Disk IO throttling (for KVM) is not applied in the merge of 4.2.
    
    Tests passed:
    (1) start vm
    (2) attach volume
    (3) start vm with volume
    (4) migrate vm (with volume)

You can merge this pull request into a Git repository by running:

    $ git pull https://github.com/ustcweizhou/cloudstack
