We're running Cocoon 2.1.4 if that makes a difference: the _validate

I have found that there is a bug in the caching of 2.1.4 such that sometimes when an exception is thrown during SAX processing, the cache validity for that 'broken' pipeline is always true.
I believe this is fixed in 2.1.5 due to the change of store provider.
(BTW There is a simple way to check if this is the same bug, make your pipelines non-caching.)
