I *am* using mod_cache, and the headers must be generated by Cocoon as the goal is for the application tell mod_cache which pages to cache and how long.
It's real easy in this way, just a sitemap action that is setting the appropriate HTTP headers.

<map:action name="cache-5" src="org.apache.cocoon.acting.HttpCacheAction">
<minutes>5</minutes>
</map:action>
