Attached patch.  Implements xpointer="..." attribute as the preferred way for 
specifying xpointer fragments.  The old href-fragments are still interpreted 
but produce a WARN log message stating support will be removed in a future 
release.  Tested against the samples and some ad-hoc tests.  Needs review.
