Resolution: Fixed
provided version removes conditional retrieval of the database resource (based
on last-modified), so can not be incorporated. Some other suggestions, such as
caching database resource component, are incorporated.
if setup() is still called twice per cocoon invocation, this is a bug in the
core which has to be fixed.
