Yes, also the issue name sometimes does not correspond to the fix itself, so it is quite good to have a separate change document, although this means a lot of work if it has not been maintained for such a long period. I find it quite natural to add a change record to changes.xml whenever I work on an issue, giving a good description of the change. So when we have a clean state it should be fine imho.
