This is a problem if you are using a proxy which lowercases all headers.
Steps to reproduce using node-http-proxy:
npm install -g http-proxy
node-http-proxy --port 1337 --target localhost:5984 &
curl -H 'Content-Type: application/json'
'http://localhost:5984/_replicator' -d
'{"source":"https://localhost:1337/test_source/","target":"test_target"}'
