It looks a lot like a bug I have with the view changes sometimes which was
solved here:
https://github.com/refuge/couch_core/commit/58bc6a23ae54f4e614638a76fd412bee7dac5c83#diff-989c52b9ea42136621dd01fbe8f7044fR548
(still in a test branch) . Imo somewhere we forget to refresh the btree state
and the information about new leafs is missing.
