I tidied this up, restoring the MAX_URI_LEN constant (since it was
repeated), introducing a minimum value beyond which we give up, and providing
an optional ability to tell couchdb to reject uri's over a certain length
(useful for testing this negotiation but also useful to align couchdb limits
with a proxy's limit).
