COUCHDB-2210 Update d3 library.
This is an update of d3.js and nvd3.js to their latest versions.
Tested for backwards compatibility with existing d3 v2 charts in dashboard
in:
* Google Chrome 33.0.1750.152
* Safari 7.0.2
