Reviewing some of this I'm realizing some of the merged code from Cloudant
relating to couch_att isn't hooked in here as it used to be. It'll probably be
a good idea to fix this up again but also enough work to warrant a separate
issue. I'll open an issue on JIRA to be handled separately.
