This code was originally factored out of most of the system but it seems like
some of the original code paths have been reverted to their old state. This
code should be ported back to couch_att and replaced with calls into
couch_att's code.
It shouldn't be too hard but it does touch multiple components. As such it will
require simultaneous pull requests across a few repositories.
