OK, I have an implementation that works in both PouchDB and PouchDB Server.
It needs to be tested against @kxepal's API somehow, though. When I try to run
this branch against the PouchDB test suite, i see various errors, including
[this one](https://gist.github.com/nolanlawson/efa5f362184af4aea3df). I have no
idea what needs to be done to fix it; I haven't been following the cassim repo
or any other fix. :)
