For exception classes without a fault bean, CXF builds the schema and
marshals the fields itself. A member-level @XmlJavaTypeAdapter on a
List or array member was not handled correctly:
- Schema: the element was generated as single-valued instead of
maxOccurs="unbounded".
- Marshal: items were written without calling the adapter. If the item
type can't be bound by JAXB this fails, the error is swallowed, and
the client receives an empty fault detail.
- Unmarshal: the adapter was never called on the items.

As in JAXB, the adapter now applies to each item, unless the member type
is assignable to the adapter's bound type, in which case the adapter
converts the whole value and the member is single-valued. Fixed for both
fields and getters.
