some references:

* the old (but still valid) way of creating/adding menu and tool bars to extensions is the Dev's Guide 4.7.3 Add-Ons, specially: http://api.openoffice.org/docs/DevelopersGuide/Components/Components.xhtml#1_7_3_3_Configuration

* the new way of merging is described in: http://wiki.services.openoffice.org/wiki/Framework/Article/Addon_Menu_Toolbar_Merging


More advanced things like complex toolbar controls, Pop up menu controller and status bar controller can be found in Carsten Driesner articles/tutorials at the Framework Project Wiki: http://wiki.services.openoffice.org/wiki/Framework#Articles_.26_Tutorials
