Emmanuel Lecharny resolved DIRSERVER-1376.

Resolution: Fixed
After a few hours debuging with Kiran, we found that an index tuple is added
every time we create an entry, containing <id,id> (this allows the engine to
know that the entry itself is a part of the search when using a SUBTREE scope),
but was never deleted.
It has been fixed.
