I have added code to prevent users from adding entries based on a selection 
from the list.  Now when item is selected, the primary key fields are disabled 
in text fields and the add button is disabled.  Pressing clear will reset these 
values.  As a next step we may want to add a 'copy' button that allows user to 
create a new entry based on values from existing entry.
