The problem turned out to be the permissions on the source file.
I changed the permissions and all is working now.

I tried to upload an image (.PNG). It uploaded (and displayed)
only part of the file, although the image size (in pixels) was
correctly reported and a box of that size was displayed. So I
tried uploading it again; this time the filesize was reported as
zero bytes, despite a "successful upload" message. I then tried
uploading a different image (also PNG) and again got a zero-byte
size. Same result with JPG image.
Am I doing something wrong, or is there a problem with the wiki?
