I can confirm that Gavins observations are correct. I just did a fresh site and
the rewrite demo works.
The remote functionality was never broken (see mail thread linked to when issue
was reported).
I think this must have been broken and fixed by some other work elsewhere
(perhaps in Cocoon?)

What do you get for localhost:8888/remoteDemo/index.html ?
I get "The requested resource "/remoteDemo/index.html" could not be found".
Sorry I should have actually tested both links rather than just stating
the remote features work so it must work here.
I see that from IRC and a few minds you all realised that the problem
was within the fresh-site not with the locationmap itself.
